Please include the following information for all fields that will receive the transferred manure. Please attach copies of the soil analyses with this form.

  • If you are not planning to land apply the transferred manure, please describe the planned end use in the space provided below. This information will need to be reviewed and approved by UVM Extension staff prior to transfer.
